What is Aftercare?

Aftercare refers to the ongoing support and services you receive after completing primary treatment. Effective aftercare creates a bridge between the intensive structure of rehab and fully independent living. Components may include step-down treatment (IOP or outpatient therapy), support group participation, sober living housing, alumni programs, individual therapy, medication management, and recovery coaching.

Why Aftercare Matters

The first 90 days after leaving treatment are the highest-risk period for relapse. Without continued support, the skills learned in treatment can fade, and old patterns can quickly resurface. Aftercare provides ongoing accountability, continued skill development, support during challenging transitions, connection to recovery community, and early intervention if relapse warning signs appear.

Types of Aftercare Programs

Intensive Outpatient Programs (IOP): Structured therapy several times per week, typically for 8-12 weeks. Standard Outpatient Therapy: Weekly or bi-weekly individual or group sessions. Sober Living: Structured, substance-free housing with peer accountability. Support Groups: 12-step meetings, SMART Recovery, or other peer support. Alumni Programs: Ongoing connection with treatment facility and fellow graduates.

Creating Your Aftercare Plan

Effective aftercare planning should begin while you are still in primary treatment. Work with your treatment team to identify your specific needs and triggers, arrange appropriate level of continued care, connect with support groups in your home area, establish housing (sober living if needed), address employment and financial needs, and build your support network.

What to Look for in Aftercare

Quality aftercare should be individualized to your needs, flexible as your recovery progresses, connected to your primary treatment, accessible and affordable, and comprehensive (addressing multiple life areas). Ask your treatment facility about their aftercare services and recommendations before you complete the program.
